Madrina literally means "godmother," but in the context of this song, it takes on a much darker, colloquial meaning: an informant or a snitch.

The lyrics, "Casi estoy seguro que fue por un dedo, Alguna madrina por aquí en San Diego," suggest that the narrator was caught due to a tip-off from an informant in San Diego. This double meaning makes the word particularly interesting and reveals a common slang usage in certain Spanish-speaking communities, especially within the genre of narcocorridos.

"El Doctor" is a narco-corrido in which the singer tells his own life story as a smuggler known by that nickname. He says he started in contraband at 14, watched trust and connections grow as he rose, and was once held for five hours by state intelligence police, only getting free after paying a ransom.

He describes pressure from the DEA and a local sheriff wanting him to write songs for them, which he refuses, and suspects an informant ("madrina") near San Diego tipped them off. He moves between Tijuana and the San Diego area (Chula Vista, San Isidro), name-checks the Xolos soccer club, and pushes back on news reports that defame him and put a price on his head.

He describes a San Judas Tadeo pendant tattooed on his chest for protection, carrying a Glock 19 for defense, and thanks his mother and wife for their support. He closes by sending greetings to friends and associates and insisting that everything in the corrido is true.
